JOB VACANCY DESCRIPTION
- Review patients’ electronic medical record, select and recruit patients to participate in clinical programmes
- Recommend investigations relevant to the evaluation of relevant diseases/illnesses
- Conduct patient education/ counselling sessions
- Coordinate doctor appointments
- Track progress of patients through phone contacts and during clinic visits at regular intervals for a period of time
- Assume responsibility for data collection and entry into system.
- Participate in interdisciplinary care team meetings and assist in developing and reviewing care plans, admissions and discharges of patients
- Any other duties as assigned
Requirements:
- Diploma or Degree in Nursing or equivalent
